Job Description
Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 2 years PT experience required License: Oklahoma Description: The physical therapist will accurately assess, differentially diagnose, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives. They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability. The therapist will work with the individual to prevent the loss of mobility. Essential functions and responsibilities include performing patient evaluations through the process of obtaining history, performing relevant systems reviews, and selecting and administering specific tests and measures to obtain data. The therapist establishes an individualized plan of care and selects treatment techniques that address the specific impairments identified in the evaluation and in accordance with physician orders. Ongoing reassessment of the patient and their response to treatment will be conducted. The therapist initiates and manages the discharge plan beginning at the first visit and continuing throughout treatment. Supervision and delegation to ancillary staff and students will be performed in accordance with state laws and department policy. Legible, accurate, and clear documentation will be completed within the established timeframe identified in department policies and procedures and in accordance with accreditation requirements and insurance regulations. The therapist communicates with the patient, family, caregivers, significant others, and members of the healthcare team to promote maximum benefit of care. Educational needs of the patient, family, caregiver, or significant other will be assessed, and education will be provided to meet those needs. Participation in professional growth and development through educational programs, including in-service meetings and workshops, is expected. The therapist actively participates in quality improvement activities. An environment conducive to safety for patients, visitors, and staff will be provided. Compliance with appropriate and approved safety standards is required. The therapist functions within the scope of practice as defined by the state’s practice act. Other related duties may be performed as required and directed.
