  • Maintain confidentiality and uphold client dignity throughout the assessment process.
  • Enter all assessments, updates, service interactions, and notes into HMIS accurately and within required timeframes.
  • Ensure assessments, ROI forms, documentation, and follows up are complete and compliant with CE and HUD standards.
  • Maintain organized service files (electronic and/or paper) following agency and CoC protocols.
  • Assist with CE data quality reviews, corrections, and verifications as needed.
  • Work collaboratively with the Morris County Continuum of Care (COC), CE partners, shelters, outreach providers and housing programs.
  • Make referrals to the emergency shelter, rapid rehousing, supportive services, or diversion resources when appropriate.
  • Communicate assessment outcomes, missing documentation, and next steps to people being served and partner providers.
  • Attend CE meetings, case conferencing meetings, and trainings required by CSPNJ or the Morris County CoC.
  • Support CE operations by helping manage assessment schedules, phone lines, or documentation requests.
