Job Description
Job Title: Physical Therapist Profession: Physical Therapist Specialty: Long Term Care Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: 2 years in Long Term Care setting License: Texas or compact license Certifications: Not specified Must-Have: 2 years PT experience in LTC setting, TX or compact license Description: The physical therapist will accurately assess, differentially diagnose, and identify deficits in a patient’s ability to move and perform functional activities in their daily lives. They develop an individualized treatment plan with patient-centered goals to reduce pain, restore function, and prevent disability. The therapist works with the individual to prevent the loss of mobility. Essential functions include: Performing patient evaluations through obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data. Establishing an individualized plan of care and selecting treatment techniques that address specific impairments identified in the evaluation and in accordance with physician orders. Conducting ongoing reassessment of the patient and their response to treatment. Initiating and managing the discharge plan beginning at the first visit and continuing throughout treatment. Supervising and delegating to ancillary staff and students consistent with state laws and department policy. Completing clear, accurate, and legible documentation within the established timeframe as per department policy and procedure. Communicating with the patient, family, caregivers, significant others, and members of the health care team to promote maximum benefit of care. Assessing educational needs of the patient, family, caregiver, or significant other and providing education to meet those needs. Participating in professional growth and development through educational programs, including in-service meetings, workshops, and reading current literature. Actively participating in quality improvement activities. Providing a safe environment for patients, visitors, and staff. Complying with appropriate and approved safety standards. Functioning within the scope of practice as defined by the state’s practice act. Performing other related duties as required and directed.
