Responsibilities Work in an inpatient hospital setting on the Case Management unit Manage an average daily census of 15 cases Responsibilities include providing and coordinating medical home care, placement, and social services for discharge planning Day shift, 5x8 hour days with 8:00 or 8:30am start times Holiday coverage required as needed Travelers are first to float within the hospital as needed and must be comfortable floating within their scope Coordinate and manage patient care for safe and effective discharges including wrap-around services First-time travelers are accepted for this position Mandatory hospital orientation modules must be completed prior to starting Return travelers must have been away for at least 6 months; former staff must be gone for 12 months before returning as travelers Most locations allow one contract extension before a six-month break from the facility is required RTO requests must be included at the time of initial consideration and cannot exceed 7 days; if more than 4 consecutive days, additional week may be added to contract Block schedule or schedule restrictions will not be accommodated Requirements Active MD or Compact RN license required at time of consideration Minimum of 2 years of inpatient case management experience Preferred experience with Epic EMR Two professional references from most recent employers required; at least one must be a supervisor or manager and the other may be a charge RN CPI certification may be required depending on the unit American Heart Association BLS required; ACLS and PALS if unit-specific NRP certification required if applicable for the unit Permanent residence must be at least 50 miles from the facility Benefits High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts American Traveler Job ID P-629050.
