Requisition Details: \u00A0 Employment Status: Temporary, Full-time (1.0 FTE)\u00A0 Program Name: Home Support Number of Hours Bi-Weekly: 75\u00A0 Work Schedule: Days Anticipated End Date: September 2026 On-Call: Yes Existing Vacancy: Yes - we're currently hiring candidates for an existing vacancy in this position. \u00A0 Job Summary: \u00A0 The Manager Home and Community Care (HCC) RPN/LPN provides professional and practice leadership to staff and is responsible for ensuring the provision of safe, effective quality client care within their program area(s). The position is accountable for working as part of an integrated Regional management team to ensure the successful execution of site operational goals and for managing the delivery of in-home client care services for Home Support programs and/or community programs within Community Support Services, including, staff hiring, supervision and training, and team operations. \u00A0 Key Responsibilities: Directs and monitors delivery of client care services and day to day functions and performance\u00A0of front-line staff in accordance with current regulatory guidelines and VON policies,\u00A0procedures and standards. Assists staff, or completes risk assessments, investigations of issues, injuries and reporting\u00A0related to client or employee incidents. Monitors performance related to funder expectations regularly and contributes to the development and implementation of quality improvement activities to achieve performance targets through monitoring of reported services. Guides new employees through the orientation process and ensures all probationary reviews\u00A0and stay interviews are completed. Applies a working knowledge of the relevant collective agreements and application of local\u00A0labour rules in daily decision-making activities. Participates in budget preparation, financial performance management. Completes reports for internal and external stakeholders conducting relevant analysis and ensuring timely submission. Completes or contributes to key performance indicators (KPI) and quality reports for\u00A0internal and external stakeholders, conducting relevant analysis and ensuring timely\u00A0submission, as required or requested In collaboration with internal stakeholders, develops team procedures and guidelines to\u00A0ensure best practices are met in the completion of all tasks or programs. Builds and sustains strategic relationships with current, new and prospective VON\u00A0partners, and community organizations. Receives and documents all employee, family and client complaints or issues related to service delivery; engages other VON managers in resolving the issue or complaint and escalates, if required, to the appropriate Director. Participates on the on-call rotation. Ensures occupational health and safety policies, programs and activities are performed by\u00A0staff, in compliance with VON\u2019s health and safety policies and guidelines and the Occupational Health and Safety Act. Participates in local, regional or national team or committees as required such as Quality Care\u00A0and Service, Joint Occupational Health and Safety, and Accreditation. Assesses and documents community needs and assists in developing business and funding\u00A0proposals to meet emerging opportunities. Provides support for department presentations and reports. Completes risk assessments, investigations of issues, incidents or injuries as required. Facilitates front line staff development and growth using a team approach and working in concert with the practice leaders, educators, preceptors and clinical experts to facilitate excellent outcomes for quality care delivery, client service and provider safety, and service provider competence. This may include but is not limited to the use of competency assessments, annual performance development plans, individual learning plans, referral to practice educators and on-going mentorship. Provides service provider personnel with access to clinical advice, support and clinical\u00A0reference resources within professional scope and in collaboration with other\u00A0regulated health professionals. Assists with the delivery of in home nursing and home support care services as\u00A0required and within the professional scope of practice. Common Leadership Responsibilities: Promotes the strategy of VON and its role as an integrated community care provider to all stakeholders and staff.\u00A0 Prioritizes and ensures alignment of emerging opportunities in the community with VON strategic plan.\u00A0 Provides leadership in navigating the requirements and complexities of unionized and non- unionized environments.\u00A0 Coaches, advises, motivates, and provides leadership to staff; creates a positive workplace for staff.\u00A0 Promotes the LEAN team management approach to ensure a common paradigm of management and discipline of management practices.\u00A0 Ensures there are mechanisms for all staff to come together to hear of organizational news and to provide feedback on what is working and what is not working.\u00A0 Ensures the completion of performance development and learning plans for all staff.\u00A0 Promotes common approaches to orientation, learning, motivating, recognizing and supporting staff when achieving organizational goals; such as meeting or exceeding KPIs, meeting or exceeding milestones.\u00A0 Provides leadership and commitment to building client and family engagement and following Accreditation Canada Standards.\u00A0 Provides leadership in the promotion of a safe and healthy workplace ensuring workplace conduct and activities are in accordance with the provincial Occupational Health and Safety Act and Regulations, that KPIs related to safety are regularly reviewed by management, that appropriate investigations occur when needed, and plans to address issues are created and acted upon.\u00A0 Compliant with the VON Safety Management System, including all Safe Work Practices and Procedures.\u00A0 Promotes work/life balance fostering professional excellence and personal fulfilment and health.\u00A0 Promotes the use of technology-based systems for the capture, transformation and distribution of knowledge.\u00A0 Abides by all confidentiality and protection of personal information policies, regulations and practices and ensures appropriate safeguards are in place in the Region.\u00A0 External and Internal Relationships: Regularly liaises/communicates proactively and constructively with various members of the home and community care and clinical management teams.\u00A0 Builds and sustains strategic relationships with current, new and prospective VON partners, and community organizations.\u00A0 Develops effective internal relationships across departments to facilitate department objectives and responsibilities.\u00A0 Interacts with funder and partner organizations such as Ontario Health at Home and Community Care Support Services (HCCSS) in Ontario. Education, Designations and Experience: Post-Secondary education in a related field.\u00A0 Registered Practical Nurse (RPN) or Licensed Practice Nurse (LPN) with current registration from provincial regulatory body, without restrictions to practice.\u00A0 A minimum of 5 years\u2019 experience in community health care and a minimum of 2 years\u2019 experience managing others.\u00A0 Experience in community work and working with community partners in a collaborative way. Experience engaging with municipal, provincial and national external groups in a relatedfield. Experience in negotiation, conflict management, financial management and working with staff\u00A0and volunteers. Experience working in a unionized environment is an asset. RPN/LPNs actively enrolled in Bachelor of Nursing program is considered an asset. Canadian Nurses Association certification or other equivalent nursing leadership program is\u00A0considered an asset. Skill Requirements: Excellent critical thinking, assessment and problem-solving skills.\u00A0 Strong attention to detail.\u00A0 Excellent verbal and written communication skills.\u00A0 Proficient in using Microsoft Office software, including PowerPoint, Word, Excel, and Outlook.\u00A0 Demonstrated ability to effectively structure daily work activities with minimal supervision.\u00A0 High level of organizational and interpersonal skills.\u00A0 Able to work under pressure in a fast-paced environment while balancing competing priorities.\u00A0 Ability to exercise innovation and identifying opportunities by practicing LEAN principles and methodologies is an asset.\u00A0 Strong public relations and networking skills.\u00A0 Demonstrated skillsets in leadership, and team building.\u00A0 Ability to accept and facilitate change.\u00A0 Other: Must have a personal vehicle or access to reliable transportation and possess both a current driver\u2019s license and proof of vehicle insurance, if applicable.\u00A0 A current and original copy of a satisfactory Criminal Records Check.\u00A0 A Vulnerable Sector Search and/or a Child Abuse Registry Check may be required.\u00A0 Willingness to travel regularly during the day as well as periodically overnight(s) to support sites and attend meetings.\u00A0 Ability to speak French is an asset in French Designated areas.\u00A0 Available to work flexible hours.\u00A0 Must be able to wear Protective Personnel Equipment (PPE) Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics.
