The Assessor Coordinator is responsible for assessment/reassessment services with ongoing client care planning and management. The Assessor Coordinator is accountable for establishing and maintaining positive liaison between the Health Centre and Home-Based Services. The Assessor Coordinator is accountable for screening, referring, reviewing, planning and facilitating safe care for individuals. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement" Human Resources Exemption: No Education Bachelor of Science in Nursing (BScN) Other Education and Training Psychiatric Nursing diploma is acceptable Nursing diploma is acceptable Bachelor of Social Work is acceptable Bachelor of Physical Therapy is acceptable Practical Nursing diploma with 4 years experience is acceptable Bachelor of Psychiatric Nursing is acceptable Bachelor of Occupational Therapy is acceptable Bachelor of Psychology is acceptable Competencies Basic - Organizational skills Negotiation skills Computer skills Problem solving skills Basic - Assessment skills Decision making skills Intermediate - Communication skills Knowledge and Abilities Knowledge in ethnic and cultural diversity Ability to work as a member of a multi-disciplinary team Knowledge of community resources Ability to prioritize and engage in empowerment dynamics with family & others.
