Attends to the daily operations of the unit on a per shift, unit level, and assumes responsibilities of a leadership role as needed. Responsibilities In a team relationship, conducts ongoing reassessment and gathers data relevant to the individual needs of the patient or family. Performs ongoing focused systems measurements, as appropriate to specialty; obtains patient history, including objective and subjective data. Collects data for the admission database per policy, in accordance with specific unit standards. Observes patient's clinical condition; reviews diagnostic information; recognizes implications of abnormal diagnostic results; identifies problems typically associated with a diagnosis or treatment; reports pertinent information to appropriate personnel for further directions. Performs individual patient care according to established plan of care and standards of practice. Identifies and documents patient/family/significant other spiritual and cultural needs.
