As our first clinical hire , you will work with high autonomy to establish the clinical protocols and culture. The Role: Advanced Home-Based Primary & Wound Care We seek an experienced, entrepreneurial Nurse Practitioner (NP) to deliver high-quality, patient-centered care directly in patients’ homes across Midland. You will independently manage a caseload of patients with complex wound care, chronic disease, and primary care needs, focusing on comprehensive, whole-patient management. This role combines advanced clinical expertise with cutting-edge technology , utilizing image-based remote monitoring to adjust care plans, accelerate healing, and drive positive outcomes. Key Responsibilities & Impact: Clinical Autonomy: Independently assess, formulate, and execute evidence-based plans of care for primary and complex wound care. This includes prescribing, ordering DME/labs, and performing sharp debridement . Technology & Triage: Utilize image-based remote monitoring to actively triage, coordinate, and adjust care plans in real-time. Mission & Growth: Serve as a clinical ambassador in the community, attending events and working directly with the owner to shape operational strategies and build strategic partnerships. Education: Provide expert education and consultation to patients, caregivers, and collaborating home health agencies to ensure continuity of care. Job Requirements / Qualifications: Nurse Practitioner (APRN) with a valid Texas license and current DEA. Minimum 4 years of advanced practice experience. At least 2 years of focused wound care experience, including sharp debridement proficiency . Proven history as a self-starter who manages a clinical schedule efficiently with minimal direct oversight. Current BLS or ACLS certification (AHA) and NPI required. Preferred: Comfort with image-based monitoring and EMRs (Eclinical Works/Athena), Bilingual (English/Spanish).
