Health and Wellbeing Coach Annual Salary: Equivalent to £31,000 (Hourly rate: £15.90) Location: Chiswick W4 Job Type: Full-time (37.5 hours per week) Join a dedicated team as a Health and Wellbeing Coach, where you will empower individuals to set and achieve health goals, focusing on lifestyle behaviours to reduce risks, improve self-management of chronic conditions, and enhance overall quality of life. This role requires flexibility to work evenings and the ability to drive to various sites including Cranford, Hounslow, Feltham, and Isleworth. Day-to-day of the role: Deliver personalised health coaching support through face-to-face, telephonic, and digital channels tailored to everyone’s specific needs, location, and preferences. Facilitate people in establishing and attaining health-promoting goals, focusing on lifestyle-related behaviours. Provide multi-channel, personalised health coaching support to individuals and groups in line with their needs and preferences. Utilise a variety of communication methods including in-app messaging, text, and email to engage participants. Advocate the use of behaviour tracking tools and share helpful resources aligned with participants' goals and stages of their journey. Appraise individuals and population caseloads daily to ensure the right intensity and frequency of support is provided. Deliver services in line with quality standards, national/government guidelines, and role boundaries. Required Skills & Qualifications: Must be able to drive and have own transport. Experience in health coaching or a related field. Proficient in motivational interviewing, goal setting, and problem-solving. Strong communication skills across multiple platforms. Ability to work flexibly, including evenings up to 19:30/20:00. Knowledge of triage and assessment, lifestyle behaviour change support, and signposting.
