Job Title: Home Health Occupational Therapist Profession: Therapy/Rehabilitation Specialty: Occupational Therapy Home Health Duration: 20 weeks Shift: Day Hours per Shift: 8 hours Experience: Minimum 1 year required, 2 preferred License: State licensed Certifications: BLS Must-Have: Must have home health experience; OASIS training preferred. Must have a valid driver's license and a car for home visits. Must be COVID vaccinated or have an exemption form on file. Must have a flu shot or declination. Mandatory mask wearing at all times while at work. Description: Provide skilled home care visits for patients with various diagnoses. Patients may include those recovering from surgery, cardiac issues, orthopedic conditions, and respiratory challenges. Deliver therapy services that focus on activities of daily living and functional independence. Utilize evidence-based practices and standardized assessments in therapy interventions. Develop individualized home exercise programs tailored to each patient’s needs. Educate patients and families on safety, injury prevention, and treatment plans. Conduct evaluations and ongoing assessments to monitor patient progress. Document therapy sessions and maintain accurate records in the charting system. Ensure compliance with all health and safety guidelines during visits. Engage in interdisciplinary collaboration to optimize patient care outcomes. Self-schedule visits based on the needs of the home care office. Perform a minimum of 6 patient visits per day, with potential for more based on discretion. Weekly patient ratios are expected to be 6-7. Remain adaptable to varying patient needs and healthcare settings. Provide support in lymphedema management, cognitive assessment, and functional capacity evaluation. Assist in discharge planning and community re-entry as needed. Foster a compassionate and supportive environment for all patients.
