Collaborate with an interdisciplinary team of therapists, nurses, and specialists. Develop and oversee individualized rehabilitation plans. Coordinate patient care transitions and ensure continuity of treatment. Participate in documentation and communication via EPIC EMR. Maintain compliance with hospital policies and credentialing standards. Deliver high-quality, compassionate patient care aligned with OSF HealthCare standards.
