Travel Occupational Therapist (OT) – Santa Clara, CA – $2,700/week Company: Per Diem Therapy Xperts (PDTX) Location: Santa Clara, CA (Skilled Nursing Facility – SNF, on-site) Schedule: Days – 5x8 hours (40 hours/week) ⏳ Contract Duration: 13 weeks (ASAP start) Weekly Pay: $2,700 (blended rate) About the Role PDTX is seeking a dedicated Travel Occupational Therapist (OT) for a 13-week assignment in Santa Clara, CA . The OT will provide patient-centered care under physician orders, helping patients restore function, improve mobility, and enhance quality of life in a Skilled Nursing Facility (SNF) setting. Key Responsibilities Evaluate and treat patients according to physician orders and individualized care plans. Develop and implement treatment plans to restore function, reduce pain, and improve mobility. Maintain accurate and timely clinical documentation in compliance with facility and state standards. Collaborate with interdisciplinary teams to ensure high-quality patient care. Educate patients and caregivers on exercises, home programs, and injury prevention. Adhere to all federal, state, and local laws, including California labor and licensure standards.
