Professional Nurse

Professional Nurse Mid Career (Above 6 yrs exp) We are seeking a Professional Nurse to provide comprehensive nursing care to patients with life-limiting illnesses. As a key member of the team, you will collaborate with interdisciplinary teams, provide emotional support to patients and families, assess referrals, manage admissions, and develop staff off-duty rosters. Deliver comprehensive nursing care to patients with life-limiting illnesses Collaborate with an interdisciplinary team to develop and implement plans Provide emotional support to patients and their families Assess and coordinate new referrals Manage admissions and discharges Plan and manage staff off-duty rosters File audits Attend Monthly meetings Cross functioning between the Inpatient Care Facility and Home Nursing program Willingness to work overtime, day and night duty, weekends and ph to meet operational requirements
