Clinical Pharmacist – Full Time (Job Share Considered) £24.45 – £28.48 per hour Hybrid working available - 3 remote/2 on site Healthii People are recruiting on behalf of a forward-thinking GP practice who are looking for a Clinical Pharmacist to join their pharmacy team. What you’ll do: Work as part of a multi-disciplinary team in a patient-facing role. Lead on areas of chronic disease management and conduct structured medication reviews. Provide clinical medicines expertise to GPs and practice staff, supporting safe prescribing and medicines optimisation. Support patients with complex polypharmacy, ensuring they get the best outcomes from their medication. Liaise with care homes, community pharmacies, and secondary care to improve medicines safety and continuity of care. Undertake audits, review prescribing data, and contribute to the development of new services. Provide training, education, and clinical support to colleagues. About you: Qualified Clinical Pharmacist with primary care experience (or working towards it). Strong communication skills and ability to work across a multi-disciplinary team. Passionate about patient care, medicines optimisation, and continuous improvement. Organised, proactive, and able to balance autonomy with teamwork.
