Hospice RN Case Manager

Job Description Job Description About Us We are a new hospice organization launching in Miami, dedicated to providing compassionate, patient-centered care with a modern approach. We are building a small founding team of experienced hospice nurses who want flexibility, autonomy, and the opportunity to shape a different kind of hospice. The Role As a Hospice RN Case Manager, you will provide skilled nursing care and support for patients and families facing life-limiting illness. This role will begin on a per-diem basis. Responsibilities Provide nursing visits for hospice patients in their homes or facilities. Perform admissions, assessments, and establish the initial plan of care. Deliver symptom management, patient/caregiver education, and emotional support. Collaborate with the interdisciplinary team, including physicians, aides, social workers, and chaplains. Document accurately and timely in the electronic medical record. Participate in IDG (interdisciplinary group) meetings as needed. Qualifications Current Florida RN license in good standing. Minimum 3–5 years of hospice nursing experience required. Strong knowledge of end-of-life symptom management and comfort care practices. Excellent communication and teamwork skills.
