Travel Nurse RN - Case Manager - $1,559 per week in Webster, NY

Job Title: Pediatric RN - Home Care Profession: Registered Nurse Specialty: Case Manager RN Duration: 65 weeks Shift: Weekend shifts Hours per Shift: 8 hours (8am - 4:30pm) Experience: One year of clinical pediatric nursing experience strongly preferred; home care experience preferred. License: Client Licensed RN Certifications: ACLS, BLS Must-Have: Certified home health care experience required; EPIC or Netsmart experience required. Description: Develops care plans appropriate to the patients' and families' health care needs in conjunction with the patient, family, physician, clinical operations team, and third-party payers. Completes the OASIS Assessment as needed to accurately reflect the clinical, functional, and social needs of the patient. Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established time frames. Refers patients to appropriate health care systems and community agencies/resources. Communicates the outcome of the assessment/reassessment visit to health care team members to ensure appropriate follow-up occurs based on the needs of the patient/family's plan of care. Displays an exemplary level of patience, courtesy, and flexibility. Interacts with patients and staff in a manner conducive to maintaining positive relationships and meeting the goals and objectives of the agency. Ensures problem resolution at the point of contact. Maintains appropriate productivity requirements. Adheres to professional home visit performance standards. Strong interviewing, assessment, and communication skills are essential. Must be able to work independently with minimal supervision, adapt to change, and manage a variety of assignments effectively. Current valid driver's license and auto insurance are required. Travel throughout the service area is expected year-round. Telemetry experience is required.
