The RN will manage a designated patient caseload, ensuring quality outcomes, timely discharge planning, efficient resource use, and accurate documentation for reimbursement. Responsibilities: Coordinate care and discharge planning across the continuum. Facilitate payor authorization and precertification processes. Collaborate with physicians, payors, and interdisciplinary teams to remove barriers to care. Support clinical documentation efforts and serve as a resource for coding accuracy. Apply process improvement methods to evaluate and improve patient outcomes. Educate patients and families regarding care plans and available resources.
