Position Overview: The ideal Licensed Practical Nurse (LPN) will have a strong background in patient care, medical documentation, and health coaching. This role involves working closely with patients in long-term care (LTC), memory care, and outpatient facilities. Key Responsibilities: Provide direct patient care under the supervision of RNs or physicians. Conduct health coaching sessions to promote wellness and manage chronic conditions. Perform accurate medical documentation in compliance with healthcare regulations. Assist in outpatient settings, LTC, and hospital environments. Administer medications and treatments as prescribed. Monitor patients’ vital signs and report changes. Support dialysis patients and educate them about the process. Use medical terminology to communicate effectively with healthcare teams. Conduct diagnostic evaluations to assess health status. Ensure infection control protocols and maintain a safe environment. Requirements: Active LPN License in VT, NH, or a compact state. Minimum 2 years of LTC experience. Ability to adapt quickly and work with minimal orientation. Experience in memory care preferred. Excellent verbal and written communication skills. Ability to work collaboratively while maintaining confidentiality. Flexibility to adapt to various healthcare environments and schedules.
