Qualified Social Worker - All Age Disability Team You will be working within a well respected Local Authority based in the Worcestershire area. You will be working in the Young Adults Team 16-25. The AAD aims to make a difference to all children and young people aged 0 - 25 with SEN and additional needs. Experience of working in transitions in essential. The Young Adults Team team support young people with disabilities aged 16-25 in Preparation for Adulthood in areas of education/employment, independent living, community inclusion and health. The work is primarily with young people with Learning Disabilities, but also some young people with a physical disability or complex health needs, to transition from Children's Services into their adult life.
