Job Description Job Description Job description Job Summary: DIRECT CARE WORKER POSITION is responsible for implementing goals established in the clients IRP, assessing client's needs and capabilities, providing supported living services, supportive counseling, skills enhancement, medication monitoring and crisis intervention to facilitate progress towards rehabilitation plan goals and integration into community life. Establishes mental and medical health linkages and utilizes available resources in the community. The DCW meets clients in their homes or in the community. Minimum Qualifications: A bachelor’s degree in a Human Services field preferred, 1 year of comparable experience; familiarity with MS Office software, professional decorum, and polished communication skills'
