Job Summary The Outpatient Counselor is responsible for providing direct clinical services to individuals affected by chemical dependency, psychiatric problems, and/or co-occurring disorders. Functions as a case manager for a patient caseload and maintains clinical records, including assessments, treatment plans, progress notes, and discharge summaries, in accordance with established time frames. Audits clinical records to ensure compliance with licensing, regulatory, and accrediting agency requirements. Communicates with families, referral sources, legal systems, and outside providers, documenting all interactions in the clinical record. Completes orientation with new patients and facilitates individual, group, and family counseling sessions. Conducts educational seminars and lectures for the patient population professionally. Completes substance use disorder assessments and insurance authorizations when necessary, and makes referral recommendations. Provides clinical outreach for patient attendance at programming, case management, or higher levels of care when needed. Prepares patient letters, reviewed by the program manager, and responds to emergency situations. Encourages patients to understand the benefits of the recommended level of care and operates as part of a multidisciplinary team. Assesses healthcare equity for current and future patient needs, collects urine drug screens, and participates in supervision, meetings, and training. Assists with coverage at other sites as necessary and ensures patient confidentiality. Education and Experience Master's degree or above in a related field with practicum experience in a health or human service agency, preferably in a drug and alcohol setting. Bachelor's degree with one year of clinical experience in a similar setting. Must hold a professional license through the PA Department of State or be credentialed through PCB, or be actively working towards licensure or certification.
