Our social workers play a vital role in supporting patients and families through counseling, mediation, case coordination, and stress-reducing interventions to ensure optimal engagement in the rehabilitation journey. What We Offer Flexibility and Autonomy : Set your own hours within a supportive structureideally available Mondays, Wednesdays, and Fridays at Tokai Estate, with potential for outpatient travel. Responsibilities: Assess Care needs Assist families to implement recommended care to ensure safe discharges are in place. Connect patients to relevant resources such as care agencies, frail cares, day clinics, palliative care resources etc. Make referrals to appropriate service providers such as psychologists, Department of Social Development etc. Provide short term counseling Provide mediation to patients and families Provide practical assistance to patients to reduce stressors in order to participate in their rehabilitation. What Were Looking For SACSSP Registration (current and valid) Professional Indemnity Insurance Training and experience in Palliative Care (essential) Additional experience in Oncology , Curatorship , or Geriatric/Frail Care (advantageous) Proven ability in Hospital Social Work and Case Management Strong report writing and time management skills Comfortable working flexible hours within normal business times Willingness to travel for outpatient consultations Ability to collaborate effectively with an interdisciplinary care team If you are a compassionate, driven social worker seeking a flexible role that values your clinical expertise , we invite you to apply and help empower patients to thrive.
