Bachelor’s degree in Nursing must be obtained within 2 years of hire. Experience and knowledge of discharge planning, case management, or utilization review processes. Knowledge of electronic health record systems. 1 year of travel experience. Description: The incumbent serves as an Outpatient/Inpatient Social Worker/Registered Nurse Case Manager. This role is responsible for direct supervision and supportive contact for identified high-risk patients. The Case Manager is responsible for designing and managing a continuum of care. The focus is on empowering clients to achieve demonstrable outcomes and self-sufficiency. Responsibilities include assessment, service planning, resource acquisition, and monitoring progress. The incumbent will link clients with community social service providers, healthcare providers, substance abuse, and mental health providers to achieve specific goals. Utilization review of patient cases in inpatient and outpatient services departments is required. The incumbent will review the medical necessity of referrals to specialty providers for services not normally provided within the facility. A multidisciplinary team approach will promote efficient and effective use of health care resources. The role aims to provide quality care, decrease fragmentation of care, enhance patient self-care and quality of life, and contain costs. The incumbent will work closely with providers, nurses, social workers, physicians, and all healthcare staff to ensure the fulfillment of utilization goals. The Case Manager will report to the Director of Case Management/Care Coordination.
