Job Description PRIDE Health is seeking a travel nurse RN Case Management for a travel nursing job in Peoria, Illinois. Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 07/07/2025 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel ID: 55190650 Shift: 8 hour days .Must be proficient in care coordination, discharge planning, and assessment of psychosocial needs/support. Must have previous experience providing care coordination/discharge planning. Specific licensures and certifications/registrations required. Case Management certification preferred, but not required. Shift or schedule requirements.8 hour shifts - M-F with rotating weekends. Call daily until 6pm. Floating to other units/departments. Could float to multiple adult units throughout the hospital General Medical, Surgical, Cardiac, Neuro, Intermediate, or ICU levels of care.
