Job Description Job Description Job Title: Home Health Nurse Essential Job Duties: Conduct Home Health Nursing Assessments to determine equipment needs and participant goals. Participate in Interdisciplinary Team (IDT) meetings to develop care plans. Deliver and document nursing interventions, ensuring a safe environment. Respond to physician orders, administer medications, and perform tests. Educate participants, families, and caregivers on disease processes and self-care. Track and monitor scheduling with the Home Health Manager. Job Requirements: Graduate of an Accredited School of Nursing. Unencumbered RN License required. Minimum of one year of nursing experience with a frail or elderly population; home health experience preferred. Reliable transportation.
