Job Title: Physical Therapist Technical Requirements and Certifications: Valid Physical Therapy Licensure Current CPR Certification Valid Driver's License Proof of Active Car Insurance Disclosure of COVID-19 Vaccination Status Job Summary: The Physical Therapist is responsible for evaluating and treating patients based on physician orders or referrals. This role involves developing individualized treatment plans to help patients achieve optimal levels of physical function and independence. The position requires effective communication with patients, families, and healthcare providers, as well as accurate documentation and compliance with all relevant regulations. Key Responsibilities: Perform comprehensive physical therapy evaluations using appropriate assessment tools and procedures. Review patient records, including diagnosis, medical history, and any precautions or contraindications. Develop measurable goals and customized treatment plans tailored to individual patient needs. Provide direct therapy interventions to support patient recovery and improvement. Communicate patient progress and needs to the interdisciplinary team, medical providers, and family members as appropriate. Consult with medical professionals regarding patient care plans, treatment progress, and discharge planning. Document evaluations, treatments, and billing in compliance with organizational, state, and federal guidelines. Supervise Physical Therapist Assistants and Physical Therapy Aides in patient care activities, ensuring adherence to state practice regulations. Participate in staff meetings and internal training programs; complete continuing education as required to maintain licensure and stay current with best practices.
