Do you have what it takes to be a Care Coordinator:

  • Completion of a Medical Assisting or LPN education program required
  • CMA/RMA or LPN required
  • Pediatrics experience required
  • Experience with quality reporting preferred

A day in the life of a HSO Care Coordinator:

  • Provide telephone outreach and in-person consultation to members identified as high risk
  • Assist in identifying gaps in care
  • Collaborate with healthcare team to optimize patient adherence to medical plan of care
  • Proactively outreach to members newly diagnosed with a targeted chronic condition
  • Educate patient and family members about preventative screenings and identify barriers to care
  • Interact with healthcare team
