Description:Monday - Friday, 40 hours per week$21.85 per hour

  • As an additional benefit for our Rostered Clinicians, we provide the required 100 hours of clinical supervision toward licensure, free of charge. Based on the current hourly rate of clinical supervision and the number of hours required, this benefit is valued at approximately $10,000.

Qualifications:

  • High School Diploma or equivalent with relevant Service Coordination experience, or a High School Diploma or equivalent and at least one year of experience in Developmental Services, or a Bachelor's degree in Human Services or a related field preferred.
  • Windows skills, typing and navigating, and e-mail usage.
  • Strong interpersonal and communication skills.
  • Valid Driver's License

SERVICE COORDINATION SERVICES: Facilitates and documents the person-centered planning process through the development, and implementation, and management of the ISA. Completes necessary documentation in a timely fashion. Assists clients in the identification of strengths, needs, and goals. Interact with assigned clients on a regular basis. Reviews ISA expectations, objectives, and data collection systems to ensure smooth, consistent implementation and that services are provided as outlined in the ISA regularly and as needed. Reviews and routes all documentation, as required. Coordinates services on behalf of the assigned individual with other service providers including but not limited to physicians, specialists, therapists, psychologists, and Aris as necessary. Conducts face to face visits at the individual's home and in their community regularly and as needed. Conducts at least one home visit per month. Adheres to all state regulations.

PROGRAM COORDINATION/PROGRAM DEVELOPMENT: Assesses individuals needs and makes recommendations as appropriate. Ensures that all waiver services are scheduled and being received by each individual. In conjunction with the clinical team members, develops individualized services for each individual. Ensures goals, objectives and record keeping systems are developed accurately written, and in place in a timely manner. Serves as liaison between agency personnel and other providers when developing services. Regularly evaluates the individual's programs for effectiveness and communicate with service providers when needed.

CLIENT ADVOCACY: Communicates with the individual's staff or other agency programs and/or other human service agencies to obtain/or provide all relevant individual information. Refers individuals to appropriate services and resources and maintains contact with all service providers. Develops resources and essential individual support services as needed. Serves as an advocate on behalf of the individual. Monitors the quality of services provided to individuals.

ADMINISTRATIVE AND NETWORKING: Responds to requests for information and documentation promptly. Maximizes the agency/division's reputation and cultivate support for agency services. Effectively represents UCS to the general public and to referral sources. Function as a representative of the agency as assigned.
